Gift Card Terms and Conditions

An Ulta Lab Tests gift card is non-refundable.
An Ulta Lab Tests gift card has no expiration date.
An Ulta Lab Tests gift card is fully transferable.
An Ulta Lab Tests gift card is electronic only and will have a unique identifier.
The holder of an Ulta Lab Tests gift card is solely responsible for security of the unique identifier and charges against the gift card using the unique identifier.
An Ulta Lab Tests gift card is not replaceable if lost, stolen, or destroyed.
An Ulta Lab Tests gift card is VOID if altered or obtained fraudulently.
An Ulta Lab Tests gift card has no value until activated. It is activated after payment clears.
An Ulta Lab Tests gift card is not a credit or debit card and has no implied warranties.
Ulta Lab Tests reserves the right, and holder or purchaser of the card agrees, that Ulta Lab Tests may at any time change the terms of the program or contract with another third party to administer the Ulta Lab Tests gift card program.
An Ulta Lab Tests gift card may not be resold under penalty of law; unlawful resale or attempted resale is grounds for seizure and cancellation without compensation.

An Ulta Lab Tests gift card can be purchased at Ulta Lab Tests with a credit card, by completing the online order form and following the directions provided.
Only forms of payment with billing addresses in the United States (and except the states of New Jersey, New York and Rhode Island) are eligible for Ulta Lab Tests gift card purchases.

The following additional rules and regulations apply to cards delivered electronically via email:
- Ulta Lab Tests is not responsible for Ulta Lab Tests gift cards that are undeliverable or not received due to inaccurate delivery information, including email addresses.
- Delivery to an incorrect but deliverable address as provided by the purchaser is the responsibility of the purchaser.

An Ulta Lab Tests gift card can be used for Ulta Lab Tests services only.
An Ulta Lab Tests gift card may only be redeemed at an Ulta Lab Tests owned and operated website.
Each purchase is deducted from the balance of the Ulta Lab Tests gift card. The balance will remain on the Ulta Lab Tests gift card until it is decreased to zero, at which time the card will be deactivated.
An Ulta Lab Tests gift card will not be redeemed or exchanged for cash, check, or credit except where required by federal or applicable state law.
An Ulta Lab Tests gift card cannot be redeemed through any third party.
An Ulta Lab Tests gift card redeemed for lab testing services is not refundable. If the tests are not completed, the funds will be held funds with an expiration date.
