  • Refundable Application Fee: A $100 fee is required upon application submission.
  • The purpose of the fee is to help prevent the misuse of our resources and ensure the sustainability of our network.
  • Refund Eligibility: You are eligible for a refund after completing 10 successful draws with orders processed by the lab within the first 12 months of your activation. This policy is designed to be fair and supportive of your transition into our network.
  • Non-Refundable Conditions: The fee becomes non-refundable if no refund request is made within 14 months or if your application is denied. This is to ensure that only committed and qualified phlebotomists utilize our network.
  • Application Requirements: The requirements are straightforward, aiming to facilitate your smooth integration into our network. We've designed them to be clear and manageable, so you can focus on your role as a phlebotomist.
  • Support: We provide support to help you succeed and integrate seamlessly into our network.

  • Due to state laws we cannot provide our service to healthcare professionals located in NJ, NY or RI.
  • You will receive an email confirming your enrollment with a personalized link for your Phlebotomist Portal that will be unique to your business.
  • After you have registered with Ulta Lab Tests we will review your information and contact you to setup your patient service center.
