Discounted Lab Tests
Ulta Lab Tests employee wellness voucher program includes employee discounted lab tests for all your employees and their families. Employees and their families receive an additional 10% off the discounted lab tests when at least 50% of your employees participate in the wellness voucher program.

Employee Coverage
Employers have the option to extend the wellness voucher program to all employees or to restrict the offering to specific employees.

Employers select from one of many popular discounted health screening panels or customize the lab tests that you would like included in the offering to your employees.

With over 2,100 patient service centers nationally, Ulta Lab Tests effectively and efficiently serves dispersed or remote employees.

Specimen collection is performed at one of 2,100 patient service centers (PSC) nationally.
Vouchers

Voucher Cost
Vouchers include the cost of the both the lab test(s) and the specimen collection.

Voucher Charges
The employer is charged only for those vouchers that are redeemed by its covered employees. No payment will be due for vouchers issued to employees who choose to not participate.

Voucher Payment
No payment is required in advance of participation by the employee. Employer is responsible for payment at the time of redemption by the covered employee.

Health Plans
Ulta Lab Tests’ discounted wellness & health screenings are great for your employees when specific health plans, premiums, or out-of-pocket contributions are based on “participation” or the results of screenings.

No Minimum Number of Participants
Most biometric and health screening vendors require a minimum number of participants or a minimum charge to provide an on-site screening event. This can make on-site screenings expensive and unrealistic. Unlike those vendors, Ulta Lab Tests can save your company money and provide additional value because there is no minimum charge or number of participants.

Reporting
Ulta Lab Tests provides employers with aggregate, de-identified data that includes participation, demographic and results data.

Limited Geographic Restrictions
Employees may participate in any state within the United States with the exception of the of NJ, NY and RI.
Results

Immediate Access
Most test results are returned overnight, generally within 12 to 48 hours after the specimen has been received by the lab. Results are available for the employee to review on their secure patient portal as soon as they are released from the processing lab.

Dynamic Review of Results
Most results are dynamically displayed via graph; they are color-coded and may be viewed by specific test, biomarker, category, body system, and time frame.

Review Changes in Biomarkers
Ulta Lab Tests’ innovative platform enables your employees to monitor their results over time, displaying up to 9 consecutive readings for each biomarker.

Biomarkers out of Clinical Range
Ulta Lab Tests’ platform enables the employee to quickly identify and focus on biomarkers that are out of clinical range and if necessary review with their physician or other designated healthcare professional.

Alerts & Notifications
Ulta Lab Tests immediately notifies the employee of any “critical value” lab test results as they are reported.

Biometric and Fingerstick Screenings
Ulta Lab Tests’ program enables your medical staff, and/or contracted medical staff, to enter the participant’s biometric data into Ulta Lab Tests’ secured platform, conduct a finger-stick blood draw, and/or collect a blood sample on a card which is then sent to Ulta Lab Tests’ contracted CLIA certified laboratory for testing.

At-home self-collection test kits
Ulta Lab Tests offers at home, self-collection tests kits that allow participants to conduct their own finger-stick collection of the blood sample on a test card which is then sent to the laboratory for processing.

Fingerstick Lab Tests

Fingerstick Kit
The employer is responsible for purchasing the individual collection test kits and distributing them to the selected employees. (The price includes contents and shipping costs, but not the lab processing fee).

Lab Test Processing Payment
No payment for the lab processing fee is required in advance of participation by the employee. The employer is charged the lab processing fee at the time of specimen collection.

Results for lab tests using Ulta Lab Tests’ fingerstick kits, where blood collection is on a card and mailed to the lab for processing, are available for your employee’s review on their secured portal as results are released from the lab. Most test results are returned overnight, generally within 12 to 48 hours after the specimen has been received by the lab.
