No. Your employees do not need to see a physician to get a lab test through Ulta Lab Tests. Ulta Lab Tests allows your employees direct access to lab tests through our healthcare professional network. Ulta Lab Tests contracted laboratories are CLIA certified and regulated by government agencies. Ulta Lab Tests allows your employees to save time and money and to share their lab results with their doctor or other healthcare provider.
Charges from Ulta Lab Tests may not be submitted for reimbursement from any insurance or other health care benefit program or payor. This prohibition applies whether the insurance program is private (such as Aetna, Blue Cross Blue Shield, United Health Care, etc.) or public (such as Medicare or Medicaid or any other state or federally funded health plan or program).
No. Your employees may not submit to Medicare or Medicaid.
Ulta Lab Tests has implemented industry best practices and controls, including state of the art technology that minimizes overhead. The result is superior customer service while providing patients with quick, accurate and secure test results. Additionally, Ulta Lab Tests affiliates with healthcare providers nationally to extend national pricing into their practices so that their patients can benefit with the cost savings of the Ulta Lab Tests program. Unlike a physician that purchases lab tests for their practice's patients, Ulta Lab Tests purchases on high testing volumes for patients across the United States. Ulta Lab Tests is able to procure national high volume pricing. Ulta Lab Tests has negotiated prompt payment with the national laboratories removing excess costs and instances where uncollectible or delay in payments could occur that would inflate the patient's price for lab tests. Securing advance payment from customers enables Ulta Lab Tests to secure exceptional costs from the national laboratories that are passed directly on to Ulta Lab Tests’ customers.
All specimens are analyzed at national CLIA certified laboratories.
Direct to consumer lab testing is prohibited under the laws of the states of NJ, NY and RI.
Abnormalities should be considered an early warning, but do not necessarily mean that your employees have an illness or disease. We strongly recommend that your employees discuss their results with their physician for evaluation, further testing and diagnosis.
Ulta Lab Tests will reach out to your employees to communicate lab tests that are identified by the lab as either “critical” or “panic” level as well as test results that are positive for HIV or STD's as required by law.
Most of the individual lab tests require no preparation. However some health and wellness panels do require fasting. If there are special requirements prior to testing it will be noted in the instructions that accompany the Ulta Lab Tests requisition form.
No. It is common to order multiple lab tests from a single blood draw where there is just one needle stick.
Yes. Your employees should always take their medicine that has been prescribed by a physician, unless the requisition instructions state otherwise.
No, at this time only credit or debit cards are accepted.
Yes. Online credit card transactions performed on the Ulta Lab Tests website are fully encrypted and secured by Authorize.net™. Once the order has been processed, the only information Ulta Lab Tests retains about the employee’s credit card is the last four (4) digits of the card's number for later reference purposes.
The charge on your employee’s credit card statement for their lab test will appear with the name Ulta Lab Tests.
After an order has been processed, your employee may cancel prior to the blood draw with a refund equal to the price of the test less a $10 cancellation fee, if cancelled within three (3) months from date of order. There is no cancellation refund after three months.
Your employees can go to the Ulta Lab Tests Contact Us page and fill out the appropriate form or they may call Ulta Lab Tests. They will need to provide their name and order number for the test that they would like to cancel.
No, the employer will only receive de-identified or non-specific aggregated data.

Yes the Ulta Lab Tests system provides discounted lab tests for all family members to purchase tests on the Ulta Lab Tests platform. Eligibility for incremental coverage or wellness screenings ordered by the employer will be determined by each employer’s arrangement with Ulta Lab Tests.
Eligibility for coverage will be determined by each employer’s arrangement with Ulta Lab Tests. Additional discounted tests may be purchased by the employee that are not covered by the employer, in which case the employee is responsible for those charges.
